The Health Information Services Department (formerly Medical Records) manages the collection and storage of all patient health information within Calvary Health Care Tasmania.
Protecting Your Privacy
The privacy of our patients and their health information is our utmost priority. Consistent with our Mission and Values, Calvary Health Care Tasmania respects and upholds our patients rights to privacy protection under the Privacy Amendment (Private Sector) Act 2000.
Access To Your Information
Requests to access the personal health information (medical record) held by Calvary Health Care Tasmania can be made by completing the Patient Access Form.

All processed requests attract a fee (capped at $35.00) to cover adminsitrative and photocopying costs.
