The Health Information Services Department (formerly Medical Records) provides services encompassing all aspects of health information management including management of the collection, use, disclosure and storage of the personal health information generated about you whilst a patient at Calvary Health Care Tasmania.
Protecting Your Privacy
We are committed to conducting our service in compliance with all applicable laws and regulations and in accordance with the highest ethical standards. We comply with the Commonwealth Privacy Amendment (Private Sector) Act 2000 and all other state legislative requirements in relation to the management of your personal information. We believe that our patients can feel safe in the knowledge that we safeguard their personal health information ensuring that confidentiality is respected and information is stored securely.
Access To Your Information
You have a right to access your personal health information and to request an amendment to the information should you believe that it is inaccurate, incomplete or out-of-date.
Requests to access your personal health information (medical record) held by Calvary Health Care Tasmania can be made by completing the Patient Access Form. Once we receive your application we have a maximum of 30 days to process your request. All processed requests attract a fee (capped at $35.00) to cover administrative and photocopying costs.
