Cardiac Rehabilitation
The Cardiac Education and Rehabilitation Program is designed to help individuals reduce the risk factors that contribute to heart disease.
This program is provided free of charge as a service by Calvary Health Care Tasmania and is recommended by your cardiologist.
The program consists of three phases:
Phase 1 - In Hospital
In Hospital
Support and education is offered to help patients and their families:
- recognise the risk factors that contribute to heart disease
- understand the anatomy and physiology of the heart and its disease
- manage ongoing care and treatment
Phase 2 - Recovery Program
After Discharge
The goal of this phase is to increase exercise tolerance and to teach you to minimise the risks of reoccurrence. This is done by offering structured educational sessions and a monitored exercise program.
This program is conducted three hours per week for a minimum of seven weeks.
The classes provide an opportunity to:
- Participate in a monitored exercise program
- Attend education sessions on a variety of relevant topics
- Socialise with other program participants while enjoying a low fat morning tea
Phase 3 - Maintenance Program
Upon completion of Phase 2 you will graduate to an independent exercise program.
Walking Groups are available in most areas including one at the Lenah Valley campus of Calvary Health Care Tasmania.
The Team
The Cardiac Rehabilitation Coordinator will meet you to discuss aspects of your heart disease, including your care after discharge.
The Dietitian helps you plan an eating pattern to meet your medical needs.
The Pastoral Care Group helps you and your family to explore ways of coping with the impact of cardiac illness.
The Pharmacist provides information about the medications you have been prescribed.
The Occupational Therapist introduces strategies to help with your lifestyle changes.
The Physiotherapist will assist your monitored exercise program and will attend to any associated muscle or joint problems.
A Former Patient will talk about their experiences and other services that are available to you in the community.
